Home Dialysis Awareness Month

Advocacy

Empowering Clinic Teams to Lead a Home-First Transformation in Dialysis Care

Why a focus on home dialysis?

Despite strong evidence and guideline support, many patients who are clinically suitable for home dialysis still default to in-centre haemodialysis. As the rates of CKD continue to rise and dialysis centres face increasing capacity and funding constraints, home dialysis offers an immediate and sustainable solution to help address these burdens in dialysis care.

During May, Home Dialysis Awareness Month provides a shared opportunity to pause, reflect and strengthen how home dialysis is positioned and delivered across the continuum of care.

Engaging during Home Dialysis Awareness Month

May offers a practical opportunity for clinicians to:

  • Revisit how and when home dialysis is introduced in modality discussions

  • Engage with peer education and clinical forums focused on real-world barriers and enablers

  • Use patient education resources to support shared decision-making

  • Reflect on how different stakeholders can better support patients dialysing at home

Education resources, event content and patient materials will be shared with clinics throughout May, allowing clinicians to engage with what is most relevant to their role and setting.

Explore relevant resources this May

  • View education and clinical discussion content related to home dialysis

  • Access patient-facing resources to support shared decision-making

  • Learn more about peer education opportunities taking place during May

Resources will be made available progressively to clinics, allowing teams to engage with content most relevant to their role and setting. To access the latest resources for your clinic, contact Vantive on 1800 920 133 (AU) or 0800 436 770 (NZ).

How this fits into routine care

Home Dialysis Awareness Month is intended as a prompt for further thinking about how all stakeholders can support patients dialysing at home.

Rather than prescribing a single approach, May supports reflection and practical action, helping to create sustainable pathways to home-based dialysis care and support informed patient choice.
